Situation

A 75 year old with poor PFTs and proven carcinoma is referred for surgery. The malignant nodule is resectable via lobectomy, not wedge. You classify the patient as pretty high risk for surgery, but doable. What do you do?

Opinion

Radiotherapy has to be considered as an option here. If the PFTs are very poor then maybe nothing as radiotherapy damages lungs.
