You open the chest to do a RLL for a 3cm nodule, proven non small cell, and unexpectedly find an involved subcarinal lymph node(N2). What would you do ?

Opinion

Probably just do a right lower lobectomy, and remove the subcarinal lymph node. The patient should probably have post operative adjuvant therapy, ? mediastinal irradiation and or chemotherapy.
