Why might this patient have haemoptysis ?

Pulmonary hypertension and on warfarin. The x-ray shows a double right heart border, and an enlarged left atrium. The patient has mitral valve disease, is on warfarin in AF.

Obviously this is a diagnosis of exclusion.
